Authorisation of the legal representative for the child’s participation

In compliance with Organic Law 15/1999, of 13 December, on Data Protection, we inform you that the personal data that may be contained in this document have been included in the files owned by Club Deportivo Espíritu Triabona. The processing of such data is for the purpose of establishing a contractual relationship with this Club.

We request knowledge in order to:

1. To participate in the Cross Golf Costa Adeje event organised by Club Deportivo Espíritu Triabona and the Sports Council of Adeje.
2. To allow those in charge of the Espíritu Triabona Sports Club to take the necessary medical-surgical decisions in the event of an emergency, under the relevant medical direction.

And I declare:

1. That I know and fully accept the Regulations of the Cross Golf Costa Adeje, organised jointly by the Department of Sports of the Town Hall of Adeje and the Espíritu Triabona Sports Club.
2. That the minor subject of this authorisation is in adequate physical condition and sufficiently prepared to participate voluntarily in this sporting event, exempting the organisation from any responsibility, knowing all the rules of the competition and accepting them in their entirety, with proof of this being the registration in the event and the signing of this document.

And I authorise:

1. To the Organization of the Cross Golf Costa Adeje, in case of any sporting injury or health incident, both in travel and in the places of celebration of the competitions, to provide appropriate medical / health care including transfer to appropriate health facilities for care.
2. To the Organisation of the Cross Golf Costa Adeje to use, for informative and promotional purposes of the race itself, any photograph, film, recording or any other form of archive of the participation of all those registered, including the minor himself, without the right to financial compensation.
